Send a concise description of the organization or program for which funding is requested. Please indicate the amount of support requested.
Provide supporting documentation including a current list of donors, current annual operating budget with income and expenses, a copy of your organization’s IRS 501(c)(3) and the names of your Board of Directors. Often your annual report will contain much of this information. Please be sure to include the name, address, phone number, and e-mail address (if available) of the primary person to contact within your organization should we have questions.

The Trustees meet in May and November of each year to conduct business and review grant requests. Please have all requests in by the end of April to be considered at the spring meeting and by the end of October for the fall meeting.
In certain cases, the Trustees may also request a tour of the agency in order to gain a better understanding of the project or program, to see first-hand who will be served, and assess the capacity of the potential grantee to carry out its objectives.
